Population Health Assessment Engine (PHATE) curriculum
The Robert Graham Center and HealthLandscape have collaborated to develop this curriculum aimed to introduce health care professionals to the Population Health Assessment Engine (PHATE).
What is the PHATE curriculum?
PHATE is a mapping tool commissioned by the American Board of Family Medicine (ABFM) that allows users to:
View community characteristics
View clinical service areas
View disease and poor quality hot spots
Connect with community organizations
Curriculum goal
The goal of the PHATE curriculum is to help learners understand how to integrate community data, clinical data, and community resources in order to address social determinants of health and improve the health of patients and populations.
How PHATE is funded
This project was funded by the American Board of Family Medicine.
Enrollment prerequisites
PHATE does not require enrollment in PRIME Registry for many of the functions in the My Community tool. The PHATE Community HotSpots tool requires data from PRIME Registry to display clusters of disease within the patient population. If not enrolled in PRIME Registry, the user will only see demo data in the Community Hotspots view.
Developed by the ABFM, the PRIME Registry is an outpatient quality data registry and Qualified Clinical Data Registry (QCDR) that includes population health and performance improvement tools for clinicians and practices. PRIME Registry extracts data from EHRs and provides a user-friendly dashboard for tracking quality measure performance, identifying patient care gaps, and automating quality reports based on that data.
PHATE curriculum modules and resources
Background 1 - Introduction to Population Health: This module introduces population health terms, concepts, and resources. Contains 70 slides.
Background 2 - Geographic and Data Concepts Important for Population Health: This module describes key geospatial terms and concepts that will be referenced in the case studies. Contains 29 slides.
Beginner 3 - Introduction to Population Health Assessment Engine: This module provides an over of what can be done using PHATE. Contains 26 slides.
Beginner 4 - How to Use My Community: This module describes how to use the My Community tool within PHATE. A synthetic data set is available so that users can practice uploading addresses to PHATE. Contains 61 slides.
Beginner 5 - How to Use Community HotSpots: This module describes how to use the Community HotSpots tool within PHATE. Contains 31 slides.
Advanced 6 - Getting the Most Out of My Community: This module describes how users can use the My Community tool within PHATE to learn about their communities. Contains 44 slides.
Advanced 7 - Getting the Most Out of Community HotSpots: This module describes how users can use the Community HotSpots tool within PHATE to learn about their communities. Contains 43 slides.
Performance improvement activity
Population Health Performance Improvement Activity: This ABFM Performance Improvement activity (PIA) instructs users on utilizing PHATE to identify social factors which may be driving gaps in patients’ health. In this activity, the user will determine whether these individuals have social needs, connect those who do with community organizations, and create new referral pathways for future patients. This PI activity can be used for certification credit.
Note: Enrollment in PRIME Registry is not necessary for the activity, but PRIME Registry enrollment does provide the ability to see clusters of disease within the physician’s patient population.
